The undersigned desire that he/she or his/her minor child, whose name is listed below, participate in activities sponsored by MARION CHURCH OF CHRIST, between the dates of OCTOBER 2019 and OCTOBER 2020. In exchange for allowing such participation I/we agree to release, forever discharge, and hold harmless MARION CHURCH OF CHRIST, its directors, officers, staff, and volunteers from any negligence of any church staff member, volunteer, or other participant in the activity that might result in the personal injury, death, medical expenses, or injury to property of the person whose names appear on this form.

By signing below, I/we specifically:
a) Assume all risks of injury to person (including death) and property and any associated medical expenses that may result from the activities sponsored by MARION CHURCH OF CHRIST;
b) Authorize MARION CHURCH OF CHRIST to furnish necessary transportation, food, and lodging to person listed below;
c) Authorize MARION CHURCH OF CHRIST to use photographs taken during the activity as it desires; and,
d) Agree to be responsible for all medical expenses that might result from injury to the person listed below.
